- A Candidate fails an examination for the second time.
- A Candidate fails to pay the annual maintenance fee of $50.
To re-establish Board Eligibility, a new Preliminary Application form and appropriate fees are required. After approval of the application, the Board will declare the individual Board Eligible. The Board Eligible Candidate may then begin the Certification process as described in the section on Procedure for Certification. Board Eligibility may be established a total of three times.
